As New York’s senior population grows, more families are looking into long-term home care as an alternative to nursing facilities. One of the most common questions we hear at Secure Safer Insurance Agency is: ๐ โDoes Medicare cover long-term care at home?โ
The short answer: Original Medicare (Parts A & B) does not cover most long-term home care servicesโbut there are important exceptions, and other options may be available to help pay for care.
Letโs break it down.

What Medicare Does Cover at Home
Original Medicare provides limited home health care under specific conditions. These include:
- Part-time skilled nursing care
- Physical, occupational, or speech therapy
- Medical social services
- Certain home health aide services (if youโre also receiving skilled care)
To qualify, you must be homebound and under a care plan created by a doctor or healthcare provider.

๐ But hereโs the key: Medicare does not pay for 24/7 care, personal care (like bathing or dressing), or long-term custodial support if thatโs the only care you need.
What Medicare Does Not Cover
Medicare does not cover:
- Ongoing help with activities of daily living (ADLs) like bathing, toileting, eating, or dressing
- Full-time home health aide services
- Meal delivery, housekeeping, or transportation
- Long-term residential care or assisted living
For these types of services, youโll need to look at Medicaid, long-term care insurance, or Managed Long-Term Care (MLTC) plans in New York.
What About Managed Long-Term Care (MLTC)?
In New York, Medicaid-eligible individuals may qualify for MLTC plans, which do cover long-term home care services such as:
- Home health aides and personal care assistants
- Nursing supervision
- Medical supplies and transportation
- Adult day care programs
To qualify, a person must be eligible for both Medicare and Medicaid, require long-term care, and live in New York State.
