Who Qualifies for Medicare Under 65?

If youโ€™re under 65, you may qualify for Medicare if:

  • Youโ€™ve been receiving Social Security Disability Insurance (SSDI) for at least 24 months
  • You have Amyotrophic Lateral Sclerosis (ALS) (automatic enrollment begins the same month your SSDI starts)
  • You have End-Stage Renal Disease (ESRD) and meet treatment requirements (typically 3 months after starting dialysis or immediately after a kidney transplant)

What Does Medicare Cover?

Once you qualify, you’ll receive the same Medicare benefits as those 65 and older:

  • Part A โ€“ Hospital stays, skilled nursing, home health, hospice
  • Part B โ€“ Doctor visits, preventive care, outpatient procedures
  • Part C (Medicare Advantage) โ€“ Optional private plan that bundles A and B, sometimes includes extra benefits
  • Part D โ€“ Prescription drug coverage

How Much Does Medicare Cost If You’re Under 65?

Most people get Part A premium-free, but you may have to pay if you haven’t worked enough quarters.
Part B has a standard monthly premium (in 2025, it’s around $185) and includes deductibles and coinsurance.

If cost is a barrier, you may qualify for a Medicare Savings Program or Medicaid in New York to help pay premiums and out-of-pocket costs.

Dual Eligibility: Medicare + Medicaid in New York

If you qualify for both Medicare and Medicaid (called dual eligible), you may get extra help covering:

  • Premiums
  • Copays and deductibles
  • Prescription drug costs
  • Long-term services and supports

What About People with Developmental Disabilities?
