If your income is roughly $22,000 or less (single), you likely qualify for Medicaid managed care in New York. Between that and roughly $36,000, you qualify for the Essential Plan — a state program that gives near-Medicaid coverage for a $0–$20 monthly premium. Above that, you enter subsidized Marketplace plans.
The catch most people miss
The Essential Plan doesn't have open enrollment windows. You can apply anytime, and coverage starts the first of the following month. This is one of the most overlooked benefits in the state.
How to actually decide
Medicaid has richer benefits (no premium, minimal copays, transportation, dental for adults in many plans). The Essential Plan is close but has slight cost-sharing on some services. If your income sits near the threshold, the safer play is to apply for Medicaid and let the state route you to the Essential Plan if you're over — the application is the same portal.
Documents to have ready
Photo ID, Social Security numbers for everyone in the household, proof of income (recent pay stubs, last year's tax return, or a self-employment income statement), and immigration status documentation if applicable.
